Introducing the Seacoast Dementia Hub: A Community Resource From RiverWoods
In 2021, a group of local caregivers who had walked the difficult and often confusing road of supporting a loved one with dementia came together to share what it felt like to search for answers, juggle services, and feel isolated in the process. Out of their similar experiences, a vision began to form. Their shared goal was to create the trusted resource they wished had existed during their own caregiving journeys. The group pictured building a place where caregivers could find clarity, direction, and support without feeling alone.
That vision became the Seacoast Dementia Hub.
Rooted in Community and Collaboration
The Seacoast Dementia Hub is grant-funded through the Foundation for Seacoast Health and hosted by RiverWoods, a not-for-profit organization with a long history of serving both its residents and the broader community. RiverWoods itself was born from the grassroots efforts of its founders, who imagined a new way to live an engaged, meaningful life with the assurance of healthcare if and when it was needed. That same spirit of reimagining how to serve the community now drives the Seacoast Dementia Hub, a program designed to explore a new model of support for caregivers and their loved ones. The Seacoast Dementia Hub reflects RiverWoods’ ongoing commitment to improving quality of life, not only for older adults but for the families and communities who care for them.
A Centralized Resource for Caregivers
The Seacoast Dementia Hub offers caregivers both emotional support and practical guidance at no cost. It helps families understand dementia and the behaviors that accompany it, build confidence in their caregiving skills, and connect with vetted local resources including respite care, adult day programs, in-home support, legal and financial experts, and medical providers. More than a service, Seacoast Dementia Hub acts as a partner, walking alongside caregivers to provide reassurance and tools that help them care for their loved ones with compassion and confidence.
How it works
The Seacoast Dementia Hub is designed to meet caregivers where they are and provide tailored support every step of the way during one of life’s most challenging crossroads. Caregivers can reach out directly via phone or email. After initial contact, a personalized intake process helps the program staff understand each caregiver’s unique situation, challenges, and goals.
Based on this assessment, caregivers are connected to the most appropriate resources. Most importantly, the program doesn’t just provide one-time recommendations, it offers ongoing follow-up, ensuring caregivers feel supported, confident, and equipped to navigate the dementia journey.
