Lamprey Health Care is a non-profit community health center that provides health care services to individuals of all ages regardless of ability to pay. We offer primary care for all ages, comprehensive prenatal and obstetrical care, an integrated behavioral health program, nutrition counseling, chronic disease care management and health education programs. We offer on-site lab services, radiology, including x-ray and mammography, and physical and occupational therapy. Additional services offered include care coordination, medication assistance program, a school-based preventive dental program, application assistance for the Health Care Market Place and NH Medicaid, and senior transportation.

Lamprey Health Care’s three centers have received Level III recognition, the highest level possible, from the National Committee on Quality Assurance. In order to receive recognition as a Patient Centered Medical Home, organizations must demonstrate that care is organized around the patient, provided by a team of health care professionals and is coordinated and tracked over time. Lamprey Health Care is proud to use the Patient Centered Medical Home model which facilitates partnerships between patients, physicians and other health care providers through the use of data, information technology and health information exchanges to ensure that patients get quality and cost effective care when and where they need it in a culturally and linguistically appropriate manner.
